A 1.5 - 2 hour comprehensive consult reviewing health, Nutritional Assessment, and dietary intake.
Service Description
This is the first step in understanding the current state of your foundational health. During this session, I will conduct a comprehensive review of your health history and assesses your current dietary intake in detail, and review of your completed online Nutritional Assessment Questionnaire (NAQ). The 1.5 hr Consult includes: -Initial health history questionnaire -Food diary evaluation -Nutritional Assessment Questionnaire (NAQ) -Educational support and guidance, targeted changes informed by your health history, diet and NAQ results. -Questions and answers addressing your specific health concerns. - Discussion of next steps, including ongoing support options and in-house clinical evaluation of your bio-individual nutrient needs to help you restore balance and optimal function in the body.
Cancellation Policy
Cancellation: Life is unpredictable, and plans can change. If you need to cancel or reschedule your appointment, please kindly let me know at least 24 hours in advance. Canceling within 24 hours of the scheduled time will incur a 50% service fee.
